BASIC FUNCTION
The Peer Support Specialist ultimately serves as a patient advocate. You will be relied on to provide clients with resourceful information and support while adhering to treatment plans in a variety of settings. The Peer Support Specialist will model competence to their clients, through their own personal experiences, in successful self-recovery. You will act as a subject matter expert while performing a wide range of tasks to support clients. The peer support will assist in transporting clients to varies places such as appointments, meetings, etc.
ESSENTIAL JOB DUTIES AND RESPONSIBLITIES
Work with clients to meet their recovery goals through lived experience while maintaining great documentation.
Build friendly and positive patient relationships by using knowledge, skills, and training from Certified Peer Specialist training to model, coach, support and advocate with patient’s.
Provide advocacy and outreach on behalf of patients in all situations where patients are unable to advocate for themselves due to symptoms of mental illness.
Provide recovery education to service recipients for every phase of the recovery journey from pre-recovery engagement, recovery initiation, recovery stabilization, and sustained recovery maintenance.
Provide a model for both people in recovery and staff by demonstrating that recovery is possible.
Assist recovering persons to identify their personal interests, goals, strengths and weaknesses regarding recovery.
Create strong relationships and outstanding one-on-one patient care services by assessing patient’s personal accomplishments, strengths and challenges in order to achieve their stated goals.
Recovery Planning -- facilitate (via personal coaching) the transition from a professionally directed service plan to a self-directed Recovery Plan. The goal should be to transition from professionally assisted recovery initiation to personally directed, community supported recovery maintenance.
Promote self-advocacy by assisting recovering persons to have their voices fully heard; their needs, goals and objectives established as the focal point of rehabilitation and clinical services.
Assist in transporting clients to appointments, meetings, etc. via company vehicles
Demonstrate ability to work cooperatively with other staff/disciplines as part of an interdisciplinary team.
Complete all required agency annual compliance trainings.
Ability to conduct educational groups regarding substance abuse / dependence.
Demonstrates knowledge of community resources for clients.
Ability to represent Stella Maris with external agencies/professionals
Maintains CPR certification.
Uphold client confidentiality and client rights.
NON-ESSENTIAL DUTIES:
Performs other duties of the position and within the individual’s capabilities as assigned by supervisor.
